Here at Shekinah Care we believe in simplicity, so in order to achieve the goals set for our Young Persons, we follow a simple three step process.

  • Pathway Plan – Agreement between Worthing & Young Person and Shekinah Care (Semi Independent Provider);

  • Care-Plan – Statement of work agreed between Young Person and Shekinah Care (Semi-Independence Provider);

  • Personal Planning (Key-working Sessions) – Reviewed quarterly, discussed weekly and implemented daily between Young person and Keyworker (Semi-Independent Provider

This three step procedure, details how we will achieve the goals set in the pathway plan for the young person.
